To become a Volunteer for Common Roads, first
you will need to download
and print the following forms and send them to us for processing:

Volunteer Information
Form:
This will help us learn about your availability, interests and
background to help us determine together what kind of volunteer
work would be best suited for you.

Voluntary
Disclosure Statement:
This is a standard disclosure form that protects both you and
us.

Staff
and Volunteer Policies:
These are the policies by which all staff and volunteers at
Common Roads abide. Please sign at the end and return the signed
section to us.
In addition, because
Common Roads serves youth, we follow the same guidelines imposed
by law for faculty and staff of Pennsylvania schools. Those
guidelines require that each employee or volunteer at Common Roads
obtain a
Pennsylvania State Police Criminal Record Check and
Pennsylvania
Child Abuse History Clearance.
Forms for both of these background checks are available for
downloading below and must be completed by all staff and
volunteers annually regardless of whether the individual has
direct contact with the youth or not. You can choose to:

Submit these forms
directly to the state agencies yourself and provide copies to
Common Roads with your volunteer application; OR
-
Send the forms to
Common Roads for us to process. If you decide to have us process
the background checks for you, please include a check for $20
made payable to Common Roads.
Please note that the state will mail the Child Abuse History
Clearance certificate directly to you. As soon as you receive it,
please forward it to us. Once we have received your completed
forms you will be contacted to meet our Executive Director or
another staff member so we can begin the process of getting
acquainted.
